بررسی شیوع سه ژن NDM-1،oqxA و oqxB در ایزوله‌های کلبسیلاپنومونیه (Klebsilla pneumoniae) جداشده از نمونه‌های ادراری

چکیده

بتالاکتاماز‌ها و پمپ‌های ترشحی از مهم‌ترین عوامل ایجاد کننده مقاومت آنتی‌بیوتیکی در باکتری کلبسیلاپنومونیه هستند. با توجه به شیوع بالای ژن‌های ایجاد کننده مقاومت و گسترش عفونت‌های بیمارستانی به‌ویژه عفونت ادراری ناشی از آن، هدف از این مطالعه بررسی میزان فراوانی ژن‌های NDM-1 ،oqxA  و oqxB در ایزوله‌های کلبسیلاپنومونیه جدا شده از نمونه‌های ادراری با استفاده از روش PCR بود. استخراج DNA با استفاده از روش جوشاندن انجام شد. واکنش PCR با استفاده ار پرایمرهای طراحی شده بر روی DNA 95 ابزوله کلبسیلاپنومونیه صورت پذیرفت. نتایج به دست آمده نشان داد که فراوانی ژن‌های کدکننده پروتئین‌های دخیل در مقاومت‌های آنتی‌بیوتیکی شامل oqxA، oqxB و NDM-1به‌ترتیب 75 ایزوله (94/78 درصد)، 75 ایزوله (94/78 درصد) و 84 ایزوله (42/88 درصد) ارزیابی شدند. نتایج این تحقیق نشان داد که شیوع ژن‌های مرتبط با مقاومت به آنتی‌بیوتیک در این مطالعه نگران کننده است و مدیریت تجویز آنتی‌بیوتیک برای کنترل عفونت و جلوگیری از انتشار باکتری‌های مقاوم و شناسایی ایزوله‌های مقاوم به دارو با استفاده از روش‌های مولکولی مانند PCR ضروری است.

Beta-lactamases and secretory pumps are the most important factors causing antibiotic resistance in Klebsiella pneumoniae bacteria due to the high prevalence of genes causing resistance and the spread of hospital infections, especially urinary tract infections caused by them, in In this study, Klebsiella pneumoniae isolates, isolated from urine samples were examined for the frequency of NDM-1, oqxA and oqxB genes using PCR technique.  DNA extraction was extracted by boiling method. PCR was done on 95 isolates of Klebsiella pneumoniae using the primers designed in this study. The showed that the prevalence of genes encoding antibiotic resistance including oqxA, oqxB and NDM-1 genes in 75 isolates (% 78.94), 75 isolates (78.94%) and 84 isolates (88.42%) respectively were evaluated. Prevalence of antibiotic resistance genes in this study is worrying and management of antibiotic prescription is necessary to control infection and prevent the spread of resistant bacteria and identify drug resistant isolates using molecular methods such as PCR.
